Job Summary
Responsible for the Sepsis Program across the System.
- Collaborates with other members of the healthcare team to set priorities for the sepsis program.
- Supports the integration and application of evidence-based practice and critical thinking skills to provide optimal healthcare outcomes.
- Coordinates performance improvement plans and processes and coordinates efforts across participating departments to meet and sustain quality and address any related issues.
- Facilitates process improvements in employee education, professional practice, systems (EHR), community awareness, patient education and overall program elements.
- Drives exceptional patient outcomes.
- Maintains knowledge in the field and is a resource to providers and clinicians enterprise wide.
- Develops system wide metric dashboard and routinely communicates outcomes to Senior and Executive Leaders
- Active participant in patient care for identified sepsis admissions (IDRs, rapid responses, patient care and patient planning of care)
- Manages the System Sepsis Committee
Minimum Qualifications
Education: Graduate of School of Professional Nursing. BSN preferred.
Experience: 3 years of experience in program management and in the care of people with chronic disease, sepsis, or critical care required.
Licensure, certification, and/or registration: Licensed as registered nurse in Commonwealth of Virginia required.
Other Minimum Qualifications: Preferred experience as RN in critical care Effective communication skills: both orally and written Computer skills Ability to work with others within a team to ensure quality patient care Strong problem-solving skills (critical thinking skills)
